Request Medical Records

Your medical record is a culmination of each separately recorded visit to each of your health care providers. Each of your providers will have a record for all episodes of care. You can request a copy of your medical records from Skyline by filling out the Release of Information form. You can either return to the front desk or return via mail/fax.
